What is the role of a community health nurse in assisting families in the care of the elderly in their homes?

The community nurse’s role in helping the elderly patient and family in the home is to coordinate care with multidisciplinary teams to optimize the care that the patient receives. Having great communication skills is imperative because often the nurse will be required to be on the phone discussing those needs with insurance companies, home care agencies, and physicians’ offices (Lancaster, 2019). The community nurse is obligated to bring evidence-based protocols to the home setting to ensure the best outcomes for the patient (Lancaster, 2019).

B) What are some of the barriers related to the initiation of home care services in communities?

One barrier to initiating home care services in rural areas is the lack of insurance or being underinsured. Many of the people in rural areas are less likely to have employer-provided health care coverage (Gorski, 2010), making it difficult to get their needs met. Having enough providers in the rural setting is also a setback.

C) How does living in an urban area versus a rural area affect home care services?

They provide skilled care so that loved ones can live longer at home and train caregivers to avoid expensive long-term care facilities. Also, they teach caregivers how to stay healthy and well.

As they care for patients at home compared to hospital or office environments, home care services in the community will result in more extended visits, which will reduce the number of patients that medical professionals can take care of per day. It is also difficult for medical professionals to include home-based treatment programs in some areas where they are naturally reluctant to visit homes in areas with high crime rates. Also, there are many barriers, such as environmental risks such as infection prevention and hygiene, trouble managing patient autonomy, and risk.
