What is the role of a community health nurse in assisting families in the care of the elderly in their homes?

The community nurse’s role in helping the elderly patient and family in the home is to coordinate care with multidisciplinary teams to optimize the care that the patient receives. Having great communication skills is imperative because often the nurse will be required to be on the phone discussing those needs with insurance companies, home care agencies, and physicians’ offices (Lancaster, 2019). The community nurse is obligated to bring evidence-based protocols to the home setting to ensure the best outcomes for the patient (Lancaster, 2019).

B) What are some of the barriers related to the initiation of home care services in communities?

One barrier to initiating home care services in rural areas is the lack of insurance or being underinsured.
